Authors Membership Summary Objectives: We have researched in a double-blind study if metformin reduces blood pressure (BP) in patients with type 2 diabetes treated intensely with insulin. Methods: A total of 220 patients with type 2 diabetes were requested to undergo 24-hour BP outpatient surveillance (24-h ABPM). One hundred and eighty-two gave informed consent. Methformin was randomized and 93 to placebo. Thirty-five subjects left (13 placebo, 22 users of metformin); 147 patients were subjected to a second 24 hour ABPM, 16 weeks after randomization. Results: Systolic BP (SBP), diastolic BP (DBP), pulse BP (PP), average BP (MP) and heart rate (HR) were measured as BP measurements of office and as ABPM of 24-hour, day and night. The measurements of Office BP did not differ significantly between placebo- and metformin groups for any BP measurement, but showed a non-significant trend for the reduction of PBS with use of metformin (adjusted mean base difference, metformin minus placebo: -4.2 mmHg, 95% CI, -9.9 to +1.5; P = 0.15). The adjusted differences in the base of the outpatient measurements were -0.2 mmHg (95% CI, -2.9 to +2.6) for the 24 hour SBP, and +1.1 mmHg (95% CI, -0.7 to +2.8) for the 24 hour DBP. In general, differences between groups treated with metformin and placebo were not statistically significant. The only significant difference was for the nocturnal PP (adjusted base line difference: -2.2 mmHg; 95% CI, -4.2 to -0.2). These results were not different after age adjustment and the duration of diabetes, or for (changes) body mass index, hemoglobin glucose, insulin dose, or plasma homocysteine. It is the first clinical trial to prove that metformin can reverse this heart condition. In their study, researchers gave 68 participants with an average age of approximately 70 or a daily dose of 2,000 mg metformin or placebo. Changes in the heart were measured by magnetic resonance (MRI). Weight, body fat, and blood pressure were also traced throughout the study. Only the metformin group experienced improved LVH symptoms, reduced blood pressure and significant weight loss, researchers reported. Low blood pressure and weight loss are two effective ways to reduce LVH effects. Researchers noted that larger trials are needed to confirm the cardioprotective role of metformin. Metformin has been on the market in the United States since 1995.
